What a support coordinator does
A good support coordinator makes your plan work harder for you. At Focus Ability Group, that means:
Helping you understand what your plan actually funds, and how the pieces fit together
Connecting you with providers who are the right fit, not just the first available option
Negotiating service agreements and handling the paperwork so you don't have to
Building your own skills and confidence to direct more of your supports over time
Stepping in when something isn't working and helping you sort it out
Two levels, depending on what you need
We provide both Coordination of Supports and Specialist Support Coordination. Coordination of Supports suits most participants: it connects you to services and builds your capacity to manage your own plan. Specialist Support Coordination is for more complex situations, where someone with additional expertise needs to help manage risk and work alongside the other professionals involved in your care.
Based in Langford, working across Perth's south-east and south metro corridor
We service Riverton and the wider south-east and south metro Perth area. Our team speaks English, Tamil, Hindi, Nepali, Bengali and Urdu between us, so language doesn't have to be another barrier.
